Overview
The Harris Center for Mental Health and IDD ("The HARRIS CENTER") is looking for a Master Level Clinician - CCAP (Midtown). Under the general direction of the Clinical Team Leader, the CCAP Clinician works collaboratively with the Midtown Management District (MMD), Harris County Constable Precinct 7, and SEARCH performing clinical activities such as independent assessments and service interventions guided by the Plan of Care and/or identified consumer need(s).
The Clinician and a Deputy Constable are partnered together daily to identify, engage, and serve individuals in Midtown, Houston who exhibit chronic homelessness and behavioral health issues. The Clinician is expected to ride along in a law enforcement vehicle daily and may be required to wear protective equipment issued by Harris County Constable Precinct 7 such as a ballistic vest.
This position interfaces with treatment teams both inpatient and outpatient and is responsible for providing:
Crisis Intervention, Behavioral Health Assessments, case management and follow-up, and liaison between the Agency and the community. The Midtown Behavioral Health Unit staff interfaces with other programs in the Agency, law enforcement, and the community.
- Perform service delivery within the community. Staff must provide direct care services during harsh weather conditions such as heat, cold, rain, and wind.
- Perform community-based crisis assessments to determine the level of care needed, assess for safety, and formulate diagnosis.
- Provide crisis counseling and intervention to individuals to reduce symptoms, provide education and prevent admission to a more restrictive environment.
- Collaboratively create a plan of care with the individual served to outline treatment goals.
- Provide homeless outreach and case management activities to assist consumers in gaining and coordinating access to necessary care and services in accordance with treatment plan goals.
- Monitor consumer progress with treatment goals and linkage to services.
- Educate family and community members so they may understand the individual’s mental health symptoms, treatment and assist with referring to community resources.
- Provide information on mental health, the importance of medications and treatment compliance, and other clinical-related information to individuals, family, and community as needed.
- Complete electronic documentation each workday as outlined in Operating Guidelines. This documentation becomes part of the medical record for the Agency.
- Follow the key principles of a trauma-informed approach (i.e. safety, trustworthiness and transparency, peer support, collaboration and mutuality, empowerment, voice and choice, cultural, historical, and gender issues).
- Meet divisional and program productivity standards.
- Perform related work as assigned and adapt to changing work requirements. The clinician must demonstrate ethical conduct in all professional activities.
- Comply with Agency policies, procedures and Operating Guidelines.
- Comply with Harris County Constable’s Office Precinct 7 policy and procedures when riding along and representing Precinct 7.
- Respond to crisis calls with a Deputy Constable. The clinician will be expected to assist the Deputy Constable with completing an Emergency Detention Order and related paperwork.
- Complete community presentations.
- Be available to provide any assistance not listed above to the Agency, Program Director, Clinical Team Lead, or staff as requested and required.
- Gain and maintain access to the Homeless Information Management System (HMIS).
- Master’s degree (M.A.) or equivalent.
- LPC-A, LMSW or LMFT-A license in the state of TX.
- Licensed (Intern) credentials plus at least 6 months of clinical experience.
- Must maintain a valid Texas Driver's License and be insurable under Agency policy.
- Must have strong diagnostic skills, the ability to incorporate patient history, symptoms, environmental and related issues to formulate proper diagnosis and treatment.
- Must have strong communication skills, listening and interpersonal skills. Written documentation must be clear, concise, and well-constructed.
