• Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology.
• Conduct outreach activities through public speaking engagements, workshops, and one‑on‑one coaching to promote healthy behaviors.
• Collect and document data related to health outcomes, service utilization, and community feedback to support program evaluation and improvement efforts.
• Collaborate with healthcare professionals, social workers, and local organizations to coordinate services for individuals with disabilities or complex health conditions.

• Demonstrated ability to work effectively with diverse populations, including individuals with disabilities or those facing socioeconomic challenges.
• Strong knowledge of medical terminology, addiction counseling principles, and nutrition fundamentals.
• Ability to build rapport quickly, demonstrate cultural competence, and maintain confidentiality at all times.
