Outpatient Occupational Therapist – PRN

We are seeking a PRN Outpatient Occupational Therapist for our in‑house rehab program.

• Licensed Occupational Therapist
• Open to applicants of all experience levels

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients per regulatory and clinical requirements.
• Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services in relation to patient evaluation; make referrals as necessary.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.
• Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
