Job Summary

Advanced Clinical Pharmacist – Critical Care/Transplant. The pharmacist will practice the profession of pharmacy with the utmost code of ethics, ensuring safe, effective, evidence‑based and cost‑effective medication selection and utilization for hospitalized patients. This role serves as a patient advocate, collaborates with multidisciplinary teams, leads pharmacy services in hospital committees, presents at conferences, publishes, and provides formal education to pharmacy and interdisciplinary teams.

Responsibilities
  • Lead committees responsible for establishing and implementing medication‑related policies and procedures (e.g., Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program).
  • Primarily oversee one or more pharmacy service lines (e.g., cardiovascular, pediatrics, critical care, etc.).
  • Demonstrate leadership within the pharmacy department through a documented record of improvements in and contributions to pharmacy practice.
  • Maintain ongoing commitment to advancing the profession in the area of pharmacy practice served.
  • Provide regular, formal education to pharmacy and interdisciplinary teams in expertise areas.
  • Model, mentor, and encourage pharmacy students, residents, and colleagues to foster highly professional pharmaceutical care and personal integrity.
  • Perform all clinical pharmacist responsibilities, including verification of Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervening when orders are unclear or potentially unsafe.
  • Interpret medication orders (verbal and written), transcribe to computerized patient medication profiles accurately, follow facility documentation procedures, and troubleshoot/clarify Centralized Order Entry (COE) orders as appropriate.
  • Prepare and dispense medications—including sterile, chemotherapy, and parenteral nutrition preparations—safely and accurately using USP 797 & 800 guidelines.
  • Evaluate Total Parenteral Nutrition (TPN) for appropriateness based on facility policies, guidelines or ASPEN recommendations.
  • Issue controlled substances to patient care areas, maintain required records, and follow corporate policies related to dispensing and monitoring.
  • Ensure safe, appropriate, evidence‑based, and cost‑effective medication therapies according to established policies, procedures, and protocols.
  • Monitor therapy regimens for contraindications and interactions, allergies, dose appropriateness, indications, and therapeutic duplications.
  • Develop, assist, implement, and execute drug therapy management protocols—including pharmacokinetics, anticoagulation dosing, IV‑to‑PO transitions, pain management, antimicrobial stewardship, and pharmacy‑directed dosing.
Qualifications
  • Required:

    Pharmacist Board Certification or Completed Residency.
  • Current pharmacist licensure.
  • B.S. or Pharm

    D. degree in Pharmacy.
  • Three years’ inpatient hospital experience preferred.
