Home Health / In-Home Physical Therapy.
Thera Care is seeking a licensed Occupational Therapist (OT) for a Full-Time or PRN home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Baytown, TX and surrounding areas.
Job Responsibilities- Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients.
- Develop and update individualized Plans of Care (POC).
- Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions.
- Address ADLs, IADLs, coordination, strength, balance, and functional mobility.
- Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments.
- Implement interventions for fall prevention, home safety, and environmental modifications.
- Train patients and caregivers in adaptive equipment and durable medical equipment (DME).
- Fabricate splints and recommend assistive devices as appropriate.
- Complete accurate and timely documentation in the EMR.
- Communicate with physicians and interdisciplinary care teams.
- Supervise COTAs and home health aides, as applicable.
- Follow all home health regulations, infection control, and safety standards.

- Doctorate in Occupational Therapy from an accredited program.
- Active Occupational Therapist (OT) license in the state of Texas or eligible Compact OT Licensure.
- Home Health experience (1 year required).
- Home Care Home Base (HCHB) experience (preferred).
- Current CPR certification.
- Strong communication, clinical judgment, and time-management skills.
- Knowledge of home health regulations and Medicare guidelines preferred.
- Valid driver’s license, reliable transportation, and current auto insurance.
