Overview

The Pharmacy Division is dedicated to ensuring safe, effective, and cost-conscious medication use while collaborating with healthcare professionals to achieve optimal drug therapy outcomes. Responsibilities
  • Clinical Pharmacy Practice:
    Develop and maintain an active clinical pharmacy practice on assigned service(s); participate in inpatient rounds and/or outpatient clinic coverage, including on-call and weekend coverage as assigned.
  • Collaborate with multidisciplinary teams to optimize pharmacotherapeutic aspects of patient care.
  • Manage medication therapy for individual patients; develop, recommend, implement, monitor, and redesign drug therapy regimens within scope of practice and institutional policy.
  • Handle daily patient medication problems and emergency situations; document interventions and notes in the electronic health record.
  • Provide timely follow-up on clinical effects of regimens with adjustments as needed; deliver drug and clinical information to healthcare professionals.
  • Develop and communicate pharmacotherapeutic plans; ensure accurate medication reconciliation and updates of medication lists.
  • Identify and resolve potential medication problems; evaluate pharmacokinetic or nutrition consults as applicable.
  • Participate in discharge planning for inpatients; assess patient finances and address barriers to therapy.
  • Guide medical staff to comply with institutional standards, including ABNs, prior authorizations, reimbursement optimization, formulary management, REMS, and drug shortage programs.
Education and Training
  • Provide effective medication- and practice-related education to patients and caregivers; share pertinent information with the healthcare team.
  • Demonstrate teaching aptitude and mastery of preceptor roles (instructing, modeling, coaching, facilitating) with residents and students.
  • Provide feedback and evaluation of resident/student performance; precept residents, students, and other trainees in clinical settings.
  • Coordinate learning experiences with residency leadership; participate in trainee activities such as grand rounds, journal club, and case conferences.
  • Deliver didactic lectures and in-services to trainees; model professional behavior and orient new clinical staff.
Clinical Support
  • Use technology to facilitate safe and efficient patient care (EHR, mobile devices, laptops, VPN).
  • Participate in processes to assess, evaluate, maintain, and improve patient care delivery.
  • Contribute to institutional initiatives to develop tools (ordersets, treatment plans, algorithms, education materials) to optimize patient care and safety.
  • Adhere to regulatory and compliance policies; complete pharmacy protocol reviews for IRB submissions where applicable.
  • Maintain required licenses and certifications (e.g., pharmacist license, BLS/ACLS, sterile compounding as assigned).
  • Document and review clinical activity and interventions; report safety events through appropriate channels.
  • Attend and participate in committees and meetings; communicate effectively verbally and in writing; align activities with departmental and institutional goals.

Other duties as assigned.

Requirements

Education: Pharm.

D. degree.

Certification: Texas State Board of Pharmacy Pharmacist license within 90 days of job entry;
Board Certification in appropriate specialty area (preferred).

Experience: Pharmacy Practice Residency (PGY1) and Residency in the area of specialization (PGY2); experience as a clinical pharmacist in the area may substitute for the required residency on a one-to-one basis (preferred).

Onsite Presence: Required.

Preferred Experience: Clinical pharmacy specialist; related experience in oncology.
