The Clinical Pharmacist Resident participates in Suvida’s non‑accredited post‑graduate residency program, a structured training experience designed to develop new‑to‑ambulatory practice pharmacists into competent clinical pharmacists within a value‑based primary care setting serving Medicare Advantage patients. Through a defined curriculum, structured topic rotations, and graduated clinical responsibility, the Resident will learn to manage complex older adults in the ambulatory setting while supporting Suvida’s clinical and quality goals.
The Resident meets patients where their needs are (virtually, in‑office, and at‑home) to identify, resolve, and prevent medication‑related problems under the supervision of a Clinical Pharmacist II preceptor. Upon successful completion of the program, the Resident may be considered for an open Clinical Pharmacist I position at Suvida, subject to availability and performance. Completion of the program does not guarantee continued employment.
Essential responsibilities consist of but are not all inclusive:
- Progress through three phases of graduated clinical autonomy over the program year, from supervised shadowing to near‑independent practice, with all clinical documentation co‑signed by a supervising Clinical Pharmacist II or higher.
- Complete topic rotations on a four‑week cycle covering geriatric ambulatory care, diabetes, hypertension/ASCVD/lipids, CHF, COPD, CKD/cardiorenal, polypharmacy and deprescribing, frailty and falls, cognitive and behavioral health, transitions of care, and ambulatory operations.
- Conduct comprehensive medication reviews while navigating multiple sources of truth (EMR, pharmacy, patient) and create and follow up on care plans with the patient, care teams, and provider team.
- Assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns.
- Optimize medication therapy to improve clinical outcomes such as blood pressure and A1c control, reduce preventable ED and hospital utilization, and monitor adherence to medication regimens and care plans.
- Collaborate with providers, interdisciplinary care teams, patients, and/or caregivers to identify and develop patient‑specific disease state therapy goals.
- Provide patient‑centered education on chronic disease states, including medications used for prevention and treatment, in a culturally responsive manner.
- Document patient encounters accurately and timely in the Elation EMR for review and co‑signature by the supervising pharmacist within required time frames.
