Position Overview
Full Time - Days - ACLS and PALS / NICU. The Pharmacy Clinical Specialist performs advanced clinical pharmacy duties to deliver optimal pharmacotherapy, facilitate safe and pharmacoeconomically rational medication use, and support pharmacy-managed protocols, formulary processes, and medication reconciliation.

Responsibilities- Conduct daily reactive and targeted proactive medication regimen reviews for patients within the designated area.
- Support pharmacy-managed protocols, formulary processes, and medication reconciliation activities.
- Provide clinical education to pharmacists, nurses, and medical staff.
- Precept pharmacy students and pharmacy residents on clinical patient care rotations.
- Collaborate with health care professionals to manage drug therapy for maximal effectiveness and minimal adverse effects.
- Document patient‑specific and non‑patient‑specific drug information support and clinical interventions.
- Manage time and prioritize tasks independently within specialty area.
- Participate in research, quality management initiatives, and continuous quality improvement projects.
- Promote a positive, team‑focused work environment and resolve problems collaboratively.
- Mentor staff pharmacists and serve as a rotation preceptor for pharmacy students and residents.
- Process orders accurately and efficiently, ensuring drug appropriateness based on diagnosis, allergies, height, and weight.
- Provide patient‑specific drug information support.
- Serve as a member on Medical Staff Committees and contribute to committee goals.
- Collaborate with health care professionals to manage drug therapy and minimize side effects, adverse drug reactions, and drug interactions.
- Provide pharmaceutical care from fundamental to advanced levels, including proactive drug therapy reviews, patient counseling, transitions of care, and pharmacist‑based dosing protocols.
- Participate in medication use evaluations, adverse drug reaction reporting, medication error reporting, and inventory management documentation.
- Provide pharmacoeconomically sound medication therapy by upholding formulary standards and avoiding medication‑related complications.
- Present in‑services and develop educational materials for pharmacy, medical, nursing, and other staff.
- Seek opportunities for continual learning and professional development.
- Contribute to design, analysis, and reporting of quality improvement findings.
- Participate in research projects and submit scholarly work for publication and presentation.
- Graduate of a pharmacy education program approved by the credentialing body.
- Successful completion of an ASHP‑accredited PGY1 residency or equivalent experience; PGY2 or fellowship preferred.
- Three years of clinical practice experience.
- Meets HM standards to become an ASHP‑Qualified Residency Preceptor within one year of employment.
- Texas Pharmacist License or eligibility to obtain it within 90 days.
- Pharmacist Preceptor Certification within 90 days.
- BLS (Basic Life Support) or Instructor certification within 90 days.
- ACLS (Advanced Cardiac Life Support) or Instructor certification within 90 days.
- PALS (Pediatric Advanced Life Support) or Instructor certification within 90 days.
- IV Certification (Intravenous) or equivalent.
- Board of Pharmacy Specialty certification.
