Job Summary
The Pharmacist Transition Specialist is responsible for actively engaging patients, fulfilling prescriptions for accuracy and safety, coordinating medication delivery and education at the patient's bedside prior to discharge, simplifying the discharge medication process, providing customer service excellence, and working closely with pharmacy, nursing, and medical staff to ensure prescriptions are filled, delivered, and education provided on time before patient discharge.
The specialist will oversee Pharmacy Technician Transition Specialists, reconcile discharge medications, perform bedside medication education, make follow-up phone calls to validate education, ensure patients are discharged on appropriate therapy, and support inpatient pharmacy operations during downtime.
Responsibilities- Engage patients actively.
- Fulfill prescriptions for accuracy and safety.
- Coordinate medication delivery and bedside education before discharge.
- Simplify the discharge medication process.
- Provide customer service excellence to all patients.
- Work closely with pharmacy, nursing, and medical staff.
- Oversee Pharmacy Technician Transition Specialists.
- Reconcile patient discharge medications.
- Perform bedside medication education.
- Make follow-up phone calls to discharged patients.
- Validate medication education and answer questions.
- Ensure all patients discharged on appropriate therapy.

Bachelor of Pharmacy:
Must have a Bachelor's Degree in Pharmacy or Pharm.
D. degree from an accredited school of Pharmacy.
Two (2) years of related pharmacy experience or PGY1 residency.
Preferred QualificationsDoctor of Pharmacy (Pharm.
D.) preferred.
- Board Certified Pharmacy Specialist (Board of Pharmacy Specialties Certification) – Preferred.
- Basic Life Support (BLS) certification within 90 days of employment.
- Pharmacist Texas Licensure or eligibility within 90 days of employment.
- Texas Pharmacist Preceptor Certification – Preferred.
- IV Sterile Certification – Required for all inpatient areas and Fondren Pharmacy at Smith Clinic;
Preferred for all other outpatient.
Bilingual Spanish – Preferred.
Communication Skills- Verbal/Written.
- Exceptional verbal (public speaking).
- Mathematics.
- Medical Terminology Knowledge.
Flexible with holidays and weekends.
Other Requirements- Professionalism, confidence, strong organizational skills, efficiency, initiative, resourcefulness, and an ability to work in a fast‑paced, rapidly changing environment.
- Excellent interpersonal skills.
- Creative problem‑solving ability.
- Advanced computer skills; experience with web‑based tools, point of sale, and clinical systems.
