Job Description SummaryThe Clinical Pharmacist is a licensed professional who provides safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the pharmacy practice professional standards and applicable regulatory entities. This entry‑level position is the first step into the practice of clinical pharmacy in the alternate setting, coordinating patient services with the Nursing Department, monitoring patient progress toward goals and educating patients, caregivers, staff and other professionals regarding appropriate medication use.
Job Responsibilities- Determine the suitability of individual patients for home care.
- Collect and organize all patient‑specific information needed to determine suitability and medical condition and prescribed medication therapy suitable for home care services.
- Assess patients' suitability for home care in accordance with Option Care policies and determine prognosis with clearly defined outcome goals.
- Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients.
- Make suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters for a home care patient when appropriate.
- Collect and organize patient‑specific information to prevent, detect, and resolve medication‑related problems and make appropriate medication therapy recommendations.
- Identify appropriate monitoring parameters for each patient and complete nutritional assessment with appropriate recommendation.
- Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information.
- Be competent in the use of pharmacokinetic principles and formulas, in assessing patient laboratory values, and in providing medication‑use education to staff, professionals, home care patients and/or their caregivers.
- Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings and use systematic procedures to communicate pertinent patient information.
- Prepare and dispense medications using appropriate techniques and following Option Care policies and procedures, including accurate calibration of equipment, proper concentration and stability, and appropriate storage.
- Adhere to safety and quality assurance practices; prepare labels that conform to the organization’s policies and include all necessary ancillary labels; inspect final medication before dispensing; ensure authorization to dispense; and comply with state/federal laws, USP standards and DEA regulations.
- Comply with policies regarding perpetual inventory systems for schedule II narcotics, controlled substances and other medications subject to diversion.
- Manage the use, maintenance, and troubleshooting of medication administration equipment and medication‑related equipment used in the management of home care patients.
- Assist in the process for assessing, managing, and reporting medication‑related incidents and equipment errors or failures; accurately complete QAR documents.
- Monitor and report discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
- Provide concise, applicable, and timely responses to requests for drug information from health care providers and home care patients, formulating responses based on literature analysis.
