Essential Responsibilities
- Complete in-home intake and admissions assessments for new clients when assignments are offered and accepted.
- Meet with clients and families to assess care needs, routines, preferences, goals, and requested services.
- Conduct a basic walkthrough of the home to identify potential fall hazards, accessibility concerns, or other factors that may affect the safe delivery of services.
- Develop individualized service plans based on the client's assessed needs, preferences, and authorized services.
- Review required intake paperwork, client rights and responsibilities, agency policies, service expectations, and other admission information with clients or their authorized representatives.
- Review approved services and rates with clients and families and refer questions involving pricing exceptions, contracts, or billing concerns to agency leadership.
- Obtain required signatures and ensure intake documentation is complete, accurate, and submitted to the agency in a timely manner.
- Communicate relevant client needs, concerns, preferences, and other information to agency leadership to support a smooth start of care and appropriate caregiver matching.
- Provide clients and families with a professional, compassionate, and welcoming experience while representing Ardenwell Home Care.
- Assist with senior expos, health fairs, community events, and other marketing or outreach activities when opportunities are available and accepted.
- Represent Ardenwell professionally when interacting with prospective clients, families, healthcare professionals, community organizations, and potential referral sources.
- Accept optional caregiver shifts when offered and based on availability, providing direct non-medical care in accordance with the client's service plan and agency policies.
- Maintain client privacy and confidentiality in accordance with HIPAA and agency standards.
- Assist with other intake, client service, outreach, or operational duties as needed and based on availability.
- Must be at least
18 years of age
. - Authorized to work in the United States.
- High school diploma or GED.
- Previous experience in home care, healthcare, senior services, customer service, social services, client relations, or a related field.
- Able to pass a
criminal background check
in accordance with Texas Health and Human Services requirements. - Able to successfully complete all pre-employment screening required by Ardenwell Home Care.
- Valid driver's license, reliable transportation, and proof of current automobile insurance.
- Ability to travel throughout Ardenwell Home Care's service area for accepted assignments.
- Strong communication, interpersonal, organizational, and customer service skills.
- Ability to work independently, gather detailed information, and complete documentation accurately.
- Basic computer and smartphone proficiency.
- Professional appearance and demeanor.
- Ability to maintain client confidentiality and handle sensitive information appropriately.
- Experience with client intake, admissions, assessments, service planning, or care coordination.
- Previous home care, caregiving, CNA, PCA, or similar experience.
- Experience working with seniors, individuals with disabilities, or their families.
- Experience with community outreach, senior expos, health fairs, or marketing events.
- Familiarity with electronic documentation or home care management software.
- CPR and First Aid certification.
- Ability to drive and travel locally to client homes and community locations for accepted assignments.
- Ability to stand, walk, bend, reach, and climb stairs as needed while completing home assessments or participating in community events.
- Ability to safely navigate a variety of residential environments.
- Ability to transport paperwork, intake materials, brochures, and other agency supplies.
- Ability to safely perform applicable direct-care duties when accepting a caregiver assignment.
- Ability to perform the essential functions of the position with or without reasonable accommodation.
