Houston Methodist seeks a compassionate Social Worker II to join our Community & Social Services team within our Healthcare & Medical Services division. This role provides comprehensive psychosocial assessment, counseling, and discharge planning for diverse patient populations, ensuring safe transitions of care and strong community linkages. Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families across inpatient and outpatient settings
- Develop and implement patient-centered care and discharge plans in collaboration with the interdisciplinary team
- Provide crisis intervention, brief counseling, and supportive therapy to address psychosocial needs
- Connect patients and families with appropriate community resources, benefits, and support services
- Advocate for patients to reduce barriers to care and promote safe transitions across the continuum
- Maintain accurate, timely documentation in the electronic health record and comply with regulatory standards
- Participate in multidisciplinary rounds, case conferences, and quality improvement initiatives
- Educate patients, families, and staff on psychosocial issues, community resources, and coping strategies
- Support hospital initiatives aligned with Houston Methodist's mission, safety, and performance goals
- Engage in ongoing professional development and contribute to a culture of continuous learning and teamwork
