Physical Therapist – Home Health

• Evaluate patients and develop individualized treatment plans
• Provide physical therapy services in the home setting
• Monitor patient progress and adjust care plans as needed
• Collaborate with physicians, caregivers, and the interdisciplinary care team
• Complete timely and accurate clinical documentation
• Supervise therapy assistants and support staff, as appropriate
• Support patients throughout the West Houston market and surrounding communities

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Valid driver's license, acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Demonstrates expertise in developing individualized treatment plans and providing physical therapy services in home settings, while effectively collaborating with interdisciplinary teams and ensuring accurate clinical documentation.
