Location:

Houston, TX 77056. Oversees medical social services under the direction of a physician.

Responsibilities
  • Conducts the Comprehensive Assessment within five (5) days of the patient’s election for hospice care to identify patients’ psychosocial, financial, environmental, and community needs through point‑of‑care documentation, clinical records, IDG/T meetings, and community resource referrals.
  • Assists with advance directives, funeral planning, discharge planning, family counseling, and changes in the level of care as indicated by the patient or family.
  • Utilizes effective interpersonal communication skills, both verbal and written.
  • Meets mandatory continuing education requirements of the Hospice and licensing board.
  • Maintains a working knowledge of public and private eligibility standards and requirements to demonstrate professional growth and competency.
  • Provides counseling to patients and family/caregivers around issues of end‑of‑life, death, and grief.
  • Promotes hospice philosophy and administrative policies to ensure quality of care.
  • Attends IDG/Team meetings, participates in the patient care planning process, and coordinates with the IDG to promote patient care collaboration.
  • Accepts clinical assignments consistent with education and competence to meet patient needs.
  • Creates an environment that respects patient privacy and property.
  • Participates in the Hospice’s QAPI program.
  • Follows the Hospice infection control policy.
  • Participates in the Hospice’s educational in‑service programs as required.
Qualifications
  • Education: Social Work (MSW) degree from a Council on Social Work Education accredited school; or Baccalaureate in social work from an accredited institution; or Baccalaureate in psychology, sociology, or related field supervised by a MSW, with one (1) year of healthcare social work experience; or Baccalaureate from an accredited school employed by the Hospice before December 2, 2008 and not required to be supervised by a MSW.
  • Licensure: Licensed as required by the state of practice. Current driver’s license. Reliable transportation and valid auto liability insurance.
  • Experience: One (1) year of experience in palliative care or hospice preferred.
  • Skills: Working knowledge of community resources. Good interpersonal skills. Demonstrated computer proficiency.
