Position Summary:

The Patient Eligibility Screener / Wellness Coordinator serves as the first point of contact for patients participating in the HGB Program. This role is responsible for reviewing patient eligibility, determining appropriate screening panels based on established guidelines, scheduling qualified patients, and providing an exceptional "white glove" patient experience from initial outreach through appointment confirmation.

This individual combines clinical knowledge with outstanding customer service to ensure patients fully understand the benefits of the program and arrive prepared for their appointments.

Primary Responsibilities:
  • Review patient information to determine eligibility for HGB screening panels.
  • Ensure patients are scheduled into the appropriate appointment types and time slots.
  • Audit provider schedules to verify appropriate patient placement.
  • Contact eligible patients to explain the screening program and available testing.
  • Schedule new patient appointments and manage rescheduling as needed.
  • Confirm appointments while educating patients on what to expect during their visit.
  • Answer patient questions regarding the screening process.
  • Maintain accurate scheduling documentation.
  • Coordinate with providers and clinic staff to maximize schedule efficiency.
  • Deliver an elevated, concierge-level patient experience throughout every interaction.
Qualifications:

Required:
  • Minimum 1 year of experience in a clinical or healthcare setting.
  • Minimum 1 year of experience working with a Medicare patient population.
  • Excellent customer service skills.
  • Outstanding verbal and written communication skills.
  • Strong organizational abilities.
  • Comfortable communicating with patients by phone.
Preferred:
  • Scheduling experience in a medical office.
  • EMR/EHR experience.
  • Experience with preventive screening programs.

Compensation & Benefits:
  • $19.00–$21.00 per hour.
