Health Navigator - Float (Job Overview)
Schedule:
Monday-Friday (8AM-5PM)

As a Health Navigator in our Clinic Float position, you will be a vital link that helps our patients access medical care, engage with their health, and stay informed about medications for HIV prevention and treatment.
This role offers you the chance to make a significant impact in the lives of individuals—and your contribution will be felt across different locations within our clinics.
- Dynamic Role: Travel between various Legacy locations to provide support and services.
- Community Engagement: Work in both clinic and community settings to identify individuals in need and connect them with essential resources.
- Empowerment Focus: Help patients navigate their health journeys, highlighting the importance of HIV/STI testing and other preventive services.
- Collaborative Environment: Become an integrated member of our clinical care team, striving for enhanced patient experiences!
- Impactful Work: Contribute to health education programming and improve overall wellness within the communities we serve.
Responsibilities Include:
- Develop and implement health education programming in one-on-one and group settings to enhance patient knowledge and wellness.
- Utilize motivational interviewing and other techniques to support patients in achieving their health goals.
- Provide information, referrals, and assistance to improve patient independence and access to services.
- Conduct point-of-care testing for HIV and other health conditions.
- Participate as a member of the interdisciplinary clinic and care team, fostering strong working relationships.
- Assess patient needs, ensuring to address behavioral, social, and medical factors.
- Establish and manage referral processes and resource lists to link patients to necessary health information.
- Promote patient-centered care within a primary care medical home context.
- Ensure a high standard of customer care to deliver positive patient experiences.
- Document all patient services accurately in electronic health records and manage patient schedules effectively.
- Meet performance metrics, especially those relating to grants and contracts.
- Embrace the spirit of teamwork as you travel between clinics to meet community needs.
- Perform additional duties as assigned to best support Legacy's mission.
- High School Diploma or equivalent
- 3 - 5 years of relevant experience.
- Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred.
- Experience performing point-of-care testing to clients in a clinical or community setting.
- Demonstrated actions and activities with respect to HIV care and prevention.
- Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
- Previous experience with Electronic Health Records Systems (EHRS).
- Ability to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire.
- Excellent communication skills and customer service skills.
- Adept in decision making responsibilities.
- Working knowledge and understanding of statistics and graphical representation principles.
- Strong computer skills (MS Office).
