Monday-Friday (8AM-5PM) | Hybrid

As a Bilingual PrEP Health Navigator, you will empower individuals by providing essential virtual patient navigation, education, and support services that emphasize HIV prevention and Pre-Exposure Prophylaxis (PrEP). You'll build meaningful relationships with patients and community members, utilizing innovative remote engagement strategies to enhance access to PrEP services, promote adherence, and support long-term care involvement.

with clinical providers, Legacy Pharmacy, and program staff to guide patients through the PrEP care continuum. - Focus on proactive strategies aimed at improving health outcomes and reducing HIV transmission risk.

- Conduct virtual outreach and engagement activities to identify, educate, and recruit individuals who may benefit from PrEP and other HIV prevention services.
- Provide remote patient navigation services to facilitate linkage to PrEP care, including scheduling appointments and coordinating with clinical teams.
- Deliver individualized health education on PrEP, nPEP, HIV prevention, medication adherence, and safer sex practices using evidence-based approaches.
- Support patient retention in PrEP care through ongoing follow-up, adherence support, and re-engagement strategies.
- Utilize motivational interviewing and goal-setting strategies to help patients overcome barriers and maintain engagement.
- Provide referrals and linkages to supportive services, including primary care, behavioral health, and community resources.
- Collaborate with interdisciplinary care teams to coordinate care and ensure a seamless patient experience.
- Document all patient interactions in the electronic health record (EHR) in a timely and accurate manner.
- Manage assigned patient caseloads and outreach activities using EHR tools and tracking systems.
- Participate in data collection, reporting activities, and quality improvement initiatives to support program goals.
- Contribute to the development and improvement of virtual education materials based on patient needs.
- Engage in team meetings and ongoing training to maintain knowledge of best practices in HIV prevention and PrEP care.
- Perform other duties as assigned.
- High School Diploma or equivalent
- 3 - 5 years of relevant experience
- Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred
- Demonstrated actions and activities with respect to HIV care and prevention
- Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
- Previous experience with Electronic Health Records Systems (EHRS)
- Ability to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire.
- Excellent communication skills and customer service skills.
- Adept in decision making responsibilities
- Working knowledge and understanding of statistics and graphical representation principles.
- Strong computer skills (MS Office).
- Demonstrated competency with an EHRS system
- Ability to manage multiple deadlines and prioritize tasks effectively.
- This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
- Ability to demonstrate core competencies of role within 90 days of hire
- Bilingual English/Spanish fluency preferred.
