Living Hope Home Health Care is a leading, established home health care provider in Houston and the surrounding areas, providing a range of services, including skilled nursing and rehabilitation care,since 2007.

• A Licensed Physical Therapist professional who provides coordinated care to patients, evaluates patients and restores or maintains patient well-being according to the plan of care
• Evaluate and provide a treatment plan for a patient with physical, psycho- socialand/or cognitive impairments
• Provide intervention to the deficit and elicit responses
• Collaborates with interdisciplinary team

• Education:
  
  Bachelor of Science in Physical Therapy
• Licensure:
  Current Texas State License as a Physical Therapist Assistant

• 1 year of minimum experience as a Physical Therapist in Home Healthcare or Acute care or Rehabilitation setting

• Therapy skills as defined as generally accepted standards of practice.
• Good communication and interpersonal skills.
• Proof of current CPR and Hepatitis profile.
• Knowledge of durable medical equipment
• Spanish is a Plus

• Perform initial and ongoing clinical assessments to identify patient needs,determine the level of function at appropriate time points and the appropriateness ofpatient for Home health
• Develop and revise a plan of care in consultation with the physician and othercare team members
• Provide therapeutic treatments, evaluate need for and use of adaptive equipment and environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable
• Evaluate outcomes of care with appropriate discharge planning.
• Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
• Accept clinical assessments that are consistent with education and competence to care for patients.
• Supervise PTA as needed
• Complete appropriate documentation in a timely manner
• Meet mandatory continuing education requirements of the agency/license board
• Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate
• Demonstrate commitment, professional growth, and competency
• Promote Agency philosophy and administrative policies to ensure quality of care
