** Position Summary
** The Community Health Worker is an integral part of the oncology care team. Community Health Worker liaises between patients and clinic personnel to connect patients to community resources and optimize a patient’s treatment journey. Community Health Workers are expected to assist with health literacy, cultural mediation, care access, and overall communication.#
** Key Performance Areas
** KPA 1 — Patient Support & Care Coordination
* Serve as a non-clinical member of the oncology care team performing independent, interdependent, and dependent support activities.
* Prepare and educate patients prior to clinic visits regarding care plans and expectations.
* Follow up with patients on health management plans and established goals.
* Develop trusting and supportive relationships with patients and families throughout the cancer care journey.
* Advocate for patient needs and help coordinate appropriate services and support.
KPA 2 — Resource Navigation & Community Linkages
* Connect patients to health-related services and community resources based on care plans and identified social determinants of health needs.
* Assist patients with accessing services, including appointment accompaniment and enrollment form support.
* Facilitate communication among patients, families, care teams, and community-based organizations.
* Promote appropriate utilization of available health and social support resources.
KPA 3 — Communication, Documentation & Compliance
* Document patient interactions, activities, and outcomes in medical record and care coordination platforms.
* Attend required care team and program meetings.
* Maintain certification through programs approved by the Texas Department of Health and Human Services.
* Demonstrate professional accountability, confidentiality, and adherence to program and organizational standards.#
