We are seeking a Registered Nurse to join our Home Health team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.

- Patient Assessment: Conduct comprehensive initial and ongoing assessments of patient status.
- Care Coordination: Collaborate with ordering physicians, therapists, and family members to develop and implement effective care plans.
- Clinical Care: Administer medications, perform wound care, and monitor vital signs, documenting all changes in condition promptly.
- Education: Empower patients and caregivers by providing education on disease management, medication adherence, and safety protocols.
- Documentation: Maintain accurate, up‑to‑date electronic medical records (EMR) in compliance with state and federal regulations.
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans.
- Administer medications and monitor patients for side effects or reactions.
- Document any changes in patient conditions and report on changes.
- Maintain accurate records of patient care and discuss observations with the supervisor.
- Currently licensed Registered Nurse (RN) in state of employment.
- Previous experience as a Registered Nurse is preferred.
- Excellent interpersonal and client care skills.
- Valid driver’s license.
- First aid/CPR certification required.
- Traveling to patient home around the Houston area.
