Overview

HM Consulting Recruiting had been hired by a Private Practice Group to seek an experienced Nurse Practitioner to join our team providing hospital inpatient and outpatient clinic services in Houston, TX 77049 area. This is a weekday, day-shift position offering structured hours with scheduling flexibility.

The ideal candidate is confident managing inpatient care, coordinating discharges, and serving as a liaison between hospital and clinic teams.

• Perform daily inpatient hospital rounding and management
• Handle mandatory discharge planning and execution
• Provide outpatient clinic coverage
• Coordinate transitions of care between hospital and clinic
• Collaborate closely with physicians and care teams
• Maintain timely and accurate EMR documentation

• Active Texas Nurse Practitioner license
• Current DEA registration
• Strong inpatient experience required
• Experience with discharge management
• Liaison or care coordination experience preferred
• Comfortable working in both hospital and clinic settings

• Competitive starting salary of $150K – $175K
