Schedule:
Full-Time, On-Call schedule is from Monday-Thursday 5p-8a.

• Respond promptly to on-call requests, triaging patient needs via phone and in-person visits.
• Assess patient condition and provide skilled nursing interventions for pain and symptom management.
• Educate patients and families on hospice care, medication administration, and comfort measures.
• Document all patient interactions accurately in the electronic medical record (EMR) per regulatory and agency standards.
• Communicate with interdisciplinary team members regarding changes in patient status.
• Coordinate with pharmacies, DME providers, and other resources as needed.

• Active RN license in the State of Texas
• Hospice 1-2yrs experience preferred
• Strong organizational, assessment, and time-management skills
• Excellent communication and customer service skills
• Valid driver’s license and reliable transportation
