Job Overview

We are seeking a compassionate, experienced Registered Nurse (RN) Hospice Case Manager to join our growing hospice team covering East Houston. This is a full-time,
in-home, field-based role providing skilled nursing care to patients in their residences and coordinating comprehensive hospice services.

The RN Hospice Case Manager will travel to patients’ homes, manage individualized care plans, and serve as a key member of the interdisciplinary team to ensure comfort, dignity, and quality end-of-life care.

The ideal candidate is organized, independent, clinically strong, and passionate about delivering high-quality in-home hospice care.

Responsibilities
  • Manage and coordinate hospice patient care in the field
  • Perform comprehensive nursing assessments and develop individualized care plans
  • Provide wound care and ongoing symptom management
  • Administer and manage medications, including patient and caregiver education
  • Coordinate with interdisciplinary team members (physicians, social workers, aides, chaplains, etc.)
  • Complete accurate and timely documentation in Kinnser (Well Sky)
  • Support patients and families with compassion and professionalism
Qualifications
  • Current, active RN license (state-specific)
  • Minimum 1 year of hospice experience (required)
  • Experience with Kinnser (Well Sky) EMR
  • Strong background in wound care
  • Experience with medication management
  • Valid driver’s license and reliable transportation
  • Ability to work independently and manage a caseload efficiently
  • Strong communication and organizational skills
Preferred Skills
  • Knowledge of hospice regulations and compliance standards
  • Experience working in home health or field-based care
