Pay Rate$28.00/hr.
- $34.00/hr.
Full Time: Monday - Friday
Part Time: Saturday and Sunday | 9:00a.m.
- 5:00p.m.
- Administer medications safely and accurately per physician orders and regulatory standards
- Oversee medication aides and conduct monthly medication reconciliation and audits
- Coordinate daily staff assignments to ensure consistent resident care
- Communicate effectively with physicians, families, and care teams regarding resident needs
- Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support
- Respond to medical and behavioral emergencies with sound clinical judgment
- Complete clinical documentation including assessments, evaluations, and service plans
- Document incidents and communicate promptly with leadership
- Monitor care team documentation for accuracy and completeness
- Support orientation and ongoing training of care staff
- Assist with scheduling in collaboration with leadership
- Attend required in‑services and continuing education
- Maintain professional appearance and hygiene
- Promote respectful and effective communication across teams and families
- Perform other duties assigned to support resident care and operational excellence
- Current and valid LVN/LPN license in the state of employment
- Basic Life Support (BLS) / CPR certification
- Minimum 6 months to 1 year experience in Assisted Living or Long‑Term Care
- Proficient in Electronic Health Records (EHR) and basic computer skills
- Strong verbal and written communication skills
- Demonstrated critical thinking and problem‑solving abilities
- Knowledge of HIPAA regulations and compliance standards
- Ability to respond with patience and understanding to dementia‑related behaviors
- Physical ability to perform essential job functions with or without reasonable accommodation
