Licensed Vocational Nurse -Bilingual; Spanish​/English

Position: LICENSED VOCATIONAL NURSE -BILINGUAL (SPANISH/ENGLISH)
The Licensed Vocational Nurse (LVN) position is a very heavily community-focused position with the overall aim of promoting community wellness through health promotion, education, screening, and licensed independent practitioner-supervised intervention activities. The Licensed Vocational Nurse assists in delivering high-quality, compassionate, patient-centered care focused on prevention, education and treatment of acute and chronic conditions appropriate for a community health care setting.

Essential Functions Provide direct nursing care including triaging of patients, taking vital signs, administering injections, immunizations and other medications.

Assist providers during examinations and procedures

Perform routine clinical procedures such as point of care (POC) testing, EKGs, and lab specimen collection

Offers education and counseling to individuals and populations in a community health setting, through various events including general health fairs, focused topic events/activations and care through mobile health units.

Serve as an integrated care team member by providing clinical advice, information, education, and counseling to clients for testing, diagnosing, and treatment of outpatient, community health diagnoses and illnesses, including sexually transmitted infection or blood-borne infections and dermatologic issues

Collaborate with local city and county municipalities for reporting and surveillance purposes of mandatory reportable illnesses and diseases.

Participate in huddles and case management activities

Work with clinical care team members to optimize clinic efficiencies

Support continuity of care across transitions including monitoring referrals and making follow-up appointments.

Provide services to patients of all ages in an appropriate, culturally safe, and judgment-free environment

Participates in El Centro’s compliance, risk, and patient safety, quality assurance, and performance improvement (QAPI) activities.

Perform supportive integrated care team tasks such as pre-visit planning, medication reconciliation, medication refills and telephone triage

Accurately document patient care in the electronic health record

Maintain confidentiality in accordance with organizational policies and procedures and HIPAA regulations

Bilingual in English/Spanish required (Written/Spoken)
An active, unrestricted Texas LVN license

At least two years of paid experience in a community health setting

Certified in CPR and AEDCurrent Basic Life Support (BLS) Certification is required.

Physical Requirements Required to sit, stand, walk, and move about throughout the day; manual dexterity to handle, or feel objects, tools, or controls; reach with arms and hands; occasionally climb stairs; balance, stoop, kneel or crouch occasionally; talk and hear; vision abilities required include close vision, distance, color and peripheral vision, depth perception and ability to focus. Occasionally lift and/or move up to 25 pounds.

Working Conditions For the most part, ambient room temperatures, lighting and a traditional office environment exist. May experience traumatic situations, including psychiatric and medical crisis situations within the environment; some exposure to hazardous materials or physical risks requiring basic safety precautions; exposure to blood borne pathogens requiring PPE; and/or exposure to viruses, diseases, and infections.
