Outpatient Registered Nurse – RN (Full‑time) – Houston, TX

Role Overview

Staff Registered Nurse (RN) within the hemodialysis team, responsible for supervising direct‑patient staff and delivering high quality care to patients undergoing chronic hemodialysis.

Duties and Responsibilities
  • Customer Service:
    • Drive the FMS culture through values and customer service standards.
    • Accountable for outstanding service to all external and internal customers.
    • Develop and maintain effective relationships through timely communication.
    • Respond to, resolve, and follow up on customer service issues promptly.
  • Staff Related:
    • Direct patient–care technicians in safe and effective delivery of chronic hemodialysis therapy per facility policy and applicable regulations.
    • Delegate tasks to direct patient–care staff (LVN/LPN, patient‑care technicians, dialysis assistants).
    • Assess daily patient–care needs and develop appropriate assignments.
    • Monitor staff techniques and adherence to policy and procedures.
    • Participate in staff training and orientation of new staff.
    • Attend all required staff meetings as scheduled.
  • Patient Related –

    Education:

    • Ensure patient and family educational needs regarding ESRD are met.
    • Provide ongoing education on renal disease, vascular access, dialysis therapy, and related health conditions.
    • Discuss diet, fluid, and medication compliance with patients.
    • Provide patient‑specific education on adequacy measures (OLC, AMP, UKM).
    • Ensure transplant and modality awareness and reduce catheter use.
    • Educate patients on laboratory values and their relationship to adequate dialysis therapy and treatment schedule.
  • Patient Related – Dialysis Treatment:
    • Deliver safe, effective care to ESRD patients.
    • Accurately implement treatment prescriptions including sodium modeling and ultrafiltration modeling where appropriate.
    • Assess response to therapy, adjust plan as directed by physician.
    • Communicate problems or concerns to Team Leader or physician.
    • Initiate and conduct initial and annual nursing assessment and ongoing documentation of patient care needs per FMC policies.
    • Participate in pre‑evaluation, initiation, monitoring, termination, access, homeostasis, and post‑evaluation processes per FMC procedures.
    • Intervene for changes in adequacy status and troubleshoot access flow issues as identified by OLC/AMP.
    • Supervise and monitor hemodialysis access care.
    • Administer and document interdialytic transfusions, notify adverse reactions to physician and blood supplier.
    • Ensure accurate documentation by Patient Care Technician on hemodialysis treatment sheet.
  • Laboratory Related:
    • Transcribe and enter physician lab orders into the Medical Information System.
    • Ensure preparation of lab requisitions for Spectra or alternate lab.
    • Use correct lab tubes and perform proper draw and processing procedures.
    • Follow up on missed labs and insufficient specimens per company policies.
    • Package specimens per DOT policies for shipment of blood or body fluids.
    • Direct labs to appropriate facilities.
    • Report abnormal lab results to physician, forward results as requested.
  • General Duties:
    • Enforce all company policies, procedures, and regulations.
    • Maintain overall shift operation safely and efficiently.
    • Act as a resource for other staff.
    • Meet regularly with Clinical Manager to discuss personnel and patient status.
    • Collaborate with physicians and other health‑team members to interpret and coordinate patient care.
    • Provide assistance with prescription refills per FMCNA policies.
    • Ensure all physician orders are entered timely.
    • Oversee documentation and maintain facility drug list.
    • Maintain competency in emergency procedures; initiate CPR and other measures as needed.
    • Ensure verification and availability of emergency equipment; provide vaccinations, immunizations, and annual TB testing.
    • Administer medications per policy; document justification for controlled substances.
    • Assist with patient transportation coordination.
  • Maintenance/Technical:
    • Maintain a clean, safe, sanitary dialysis treatment area.
    • Demonstrate competency in operating dialysis equipment.
    • Keep patient stations and machines clean and appropriately positioned.
    • Respond to blood spills per Bloodborne Pathogen Control Policies.
