The ideal candidate will have advanced clinical education, relevant experience in oncology or palliative care, and appropriate licensure as an Advanced Practice Registered Nurse, along with demonstrated expertise in communication, interdisciplinary collaboration, and patient-centered decision support.
Minimum Salary $117,500 | Midpoint Salary $146,500 | Salary Maximum $176,500

- Identify patients nearing the end of life across inpatient and outpatient settings
- Deliver comprehensive therapeutic patient care aligned with evidence-based practices
- Integrate patient values and preferences into individualized care plans
- Coordinate and facilitate high-quality goals of care conversations with patients and families
- Collaborate with inpatient and outpatient care teams to ensure aligned communication
- Support decision-making regarding advance directives, CPR, intubation, and artificial nutrition
- Guide discussions on preferred setting for end-of-life care
- Partner with primary outpatient cancer providers and inpatient teams to reach consensus on prognosis
- Communicate prognosis clearly and compassionately to patients and families
- Coordinate care planning across inter‑professional teams to ensure continuity
- Educate patients and families on treatment options and expected outcomes
- Provide emotional support to patients and caregivers during critical decision‑making
- Advocate for care plans that prioritize comfort and quality of life
- Ensure accurate and timely documentation in the medical record
- Participate in data collection to support program evaluation and quality initiatives
- Collaborate with clinical and operational leadership to advance the Goal Concordant Care Initiative
- Required:
Master's Degree Nursing or Doctorate Nursing Practice. - Degree may be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003.
