• During rounds, observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conducts rounds with physician and records visits in clinical records.
• Performs narcotic reconciliation with incoming and outgoing shifts.
• Identifies significant changes in resident condition and takes necessary action per facility policies and procedures.
• Oversees and monitors aides.
• Initiates and completes incident and accident reports per facility policy.
• Discharges residents per facility policy.
• Completes nursing assessment for physician calls prior to calling physician on any change in resident condition.
• Communicates with residents’ point of contact when they ask for a status update or if there is a change in condition.
• Other special projects or functions assigned by the Director of Nurses and/or Administrator.

• Has completed an LVN program at an accredited vocational school or college.

• Must be an active Licensed Vocational Nurse in the state of Texas in good standing.

• Experience working in a skilled nursing facility is preferred.

Physical demands include regularly lifting, carrying, pushing, pulling, sliding/transferring, and other tasks. The employee may lift up to 50+ lbs, carry up to 50+ lbs, and perform frequent tasks such as sliding/transferring residents up to 50+ lbs. The job also involves standing, sitting, walking, speaking, driving, balancing/climbing, stooping/kneeling, crouching/crawling, reaching, hearing/listening, seeing, and turning/twisting/leaning. Physical demands occur frequently and are essential for successful performance of the job.
