Referral Management LPN
This role is a PRN, variable hour shift position that reviews assessment referrals and collaborates with RN and other clinicians to determine appropriate placement of patients seeking treatment. The LPN assists the call center specialist to enhance the referral management process.
Job Duties /Responsibilities- Review assessment referrals received and collaborate with RN to assess patient needs.
- Assist with coordination of patient care.
- Maintain patient records, updating databases, and ensuring accurate documentation of patient information.

Nursing Degree from Vocational/Technical School, preferred Associate’s Degree in Nursing.
Licensure/CertificationLicensed LPN State of Florida or compact license. Current CPR if required by a customer hospital.
Experience1-3 years related work experience.
