- Provide care based on licensure/certification within scope of practice.
- Maintain an updated clinical record on each patient at all times, meeting required documentation deadlines.
- Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
- Consult with physicians regarding alterations of patient care plans and report all changes to the nurse supervisor.
- Support FFHC’s mission by mentoring or coaching new employees during patient‑specific orientations.
- Educate patients and their caregivers on topics related to patients’ care needs.
- Participate in FFHC Quality Assurance, Performance Improvement program as requested.
- Continuously strive to improve nursing care skills.
- Attend and complete all mandatory training/in‑services as assigned.
- Submit required personnel file documentation in a timely manner.
- Adhere to FFHC compliance and ethics expectations.
- Maintain patient and family confidentiality.
- Maintain professional, supportive, and responsive interpersonal communication skills.
- Perform other duties as assigned.
- LPN/LVN Nursing Degree.
- Unencumbered LPN/LVN License in the state of employment or multi‑state license through the Enhanced Nurse Licensure Compact where recognized.
- One year of demonstrated experience.
- Valid driver’s license with proof of insurance in applicable states.
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Valid and current CPR card/certificate with in‑person instructor‑led hands‑on skills check.
- Clear Hepatitis B screening in applicable states.
- Clear TB screening in applicable states.
- State‑specific knowledge (e.g., Florida, Illinois, Pennsylvania, Texas requirements).
- Additional background screening such as Child Abuse or FBI check.
- Completion of TB screening with 2‑step PPD or proof of completion within the last year.
Florida: Valid driver license with proof of insurance is not required; staff are prohibited from driving patients. Hep B screening is not a prerequisite for hire and is completed during onboarding.
Illinois: Valid driver license with proof of insurance is not required.
Pennsylvania: Additional background screening (Child Abuse, FBI check) and TB screening with 2‑step PPD or proof of 1‑step PPD are required.
Texas: No additional state‑specific requirements noted.
Key Competencies- The ability to read and communicate in English both verbally and in writing.
Work is performed in patient homes and is not substantially exposed to adverse environmental conditions. Training may be performed in an office setting. Must be able to lift up to 35 pounds without an assisted lifting device. Frequent use of office machines including telephone, computer, and printer.
