Key Responsibilities:
- Collaborate with various departments to integrate care and treatment.
- Manage patient care under the Patient-Centered Medical Home Model.
- Conduct comprehensive assessments and develop treatment plans.
- Prescribe medications and provide counseling on health maintenance.
- Ensure timely completion of patient charts and maintain an average of 20 encounters daily.
