Job Summary
The Advanced Practice Nurse, Hospital at Home (APN‑HaH) is accountable for providing comprehensive patient care congruent with the role and approved population (Family/Individuals across lifespan and Adult/Gerontology) through systemic assessment, diagnosis, treatment, management, and follow‑up affecting direct patient care under the supervision of the HaH Clinical Medical Director. The APN‑HaH serves as a clinical expert and consultant to support the healthcare system in providing patient‑centered, culturally‑competent, outcome‑focused therapeutic care within the management of specified populations through independent and collaborative clinical management.
The APN‑HaH aligns practice with professional rules and regulations, accreditation standards, and organizational policies and procedures.
This APN‑HaH provides high‑quality patient care to various patient populations within the framework of the Hospital at Home Program model. The APN‑HaH will be accountable for planning, implementing, evaluating, and communicating all phases of the patient’s plan of care to the medical and nursing teams. This APN is responsible for facilitating an environment that supports professional practice and empowers all interdisciplinary health‑care workers to provide safe, effective, compassionate, and efficient patient care.
Degrees- Bachelor’s Degree
- Master’s Degree:
Graduation from an accredited School of Nursing with a Master’s degree in Nursing with completion of a university‑based program for Advanced Practice Nurse required. Acute Care NP or Adult‑Gerontology Acute Care NP preferred
- Current specialty certification from nationally recognized certifying organizations
- Current prescriptive authorization (or eligibility) number from the Texas Board of Nursing (TX BON)
- ANCC Certification
- DEA:
Current and unrestricted Drug Enforcement Registration (DEA) License (or will be required to obtain have a DEA registration license within 3 months of hire) (Nurse Practitioner) - Advanced Practice Registered Nurse:
Current recognition as APN with the Texas Board of Nursing (TX BON) - Registered Nurse:
Current RN license with the Texas Board of Nursing (TX BON) - Basic Life Support:
Current Basic Life Support Certification from an American Heart Association or Red Cross approved program - Advanced Cardiac Life Support:
Current Advanced Cardiac Life Support Certification from American Heart Association (as appropriate to specialty area) - Valid TX Driver's License
