In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - SaintCloud, MN

Overview

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations for members across the United States. The position is 40‑hour per week, with a pay range of $41.85 to $90.13 per hour depending on experience and location.

• Conduct in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review and physical examination.
• Coordinate diagnostic or preventive tests such as diabetic eye exams, spirometry, bone density screening, and other tests ordered by the member’s health plan.
• Use an iPad and an intuitive clinical workflow to complete evaluations; do not prescribe medication, order lab tests, or alter current treatment regimens unless otherwise directed.
• Build trusted relationships with members and provide education to help them make informed health decisions.
• Conduct virtual visits as needed.
• Travel locally and within licensed states, with priority coverage of Saint Cloud and surrounding areas.
• Provide all Signify Health services as indicated, including diagnostic and preventive services products.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required for both NP and PA tracks.
• Multi‑state licensure (preferred).
• Ability to work safely in private residences that may contain pets, secondhand smoke, or airborne allergens.
• Strong verbal and written communication skills and a patient‑centered approach.

The primary workplace is the member’s private residence. Clinicians must be comfortable performing duties in diverse home settings and be prepared for potential allergens or irritants.
