Job Summary

Common Spirit Health at Home is recruiting RN Home Health Care in Houston, TX. This is a full‑time day‑shift role (Mon‑Fri, 8:00 AM‑5:00 PM) with no obligatory after‑hour on‑call obligations and one mandatory weekend on‑call rotation per month.

• Act as a clinical leader by delivering high‑quality, personalized nursing care in the patient’s home environment.
• Develop, implement, and dynamically adjust detailed care plans that promote patient independence and well‑being.
• Collaborate and coordinate with physicians, therapists, and support staff to ensure holistic care.
• Provide critical education and emotional support to patients and their families.
• Mentor and guide LPNs, LVNs, and Home Health Aides, fostering teamwork and professional development.
• Utilize advanced technology and documentation systems to improve care delivery and maintain accurate records.

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of one year of nursing experience; mandatory experience in Home Health is preferred.
• Excellent critical thinking, clinical expertise, mandatory case‑management skills, and mandatory commitment to patient‑centered care.
• Mandatory current CPR/BLS certification.
• Mandatory valid driver’s license and reliable transportation.
