In-Home Nurse Practitioner or Physician Assistant Per Diem - Cincinnati

Job Overview

Aperd a per diem clinician (Nurse Practitioner or Physician Assistant) with Signify Health proficient in delivering in‑home health evaluations and bridging care gaps. Visits are approximately 45 minutes and involve medication and medical history review, physical assessment, and optional diagnostic tests such as diabetic eye exam and spirometry.

• Conduct 45‑minute home visits for health plan members, reviewing medication and medical history, performing physical assessment, and ordering required tests.
• Use an iPad and an intuitive clinical workflow to document findings.
• Provide Diagnostic & Preventive Services including spirometry, diabetic eye exam, bone density screening, and related items as selected by the health plan.
• Offer virtual visits as required by the health plan.
• Maintain compliance with all regulatory requirements and Signify Health sign‑off.

Work is performed in the private residence of the member. Clinicians must be comfortable operating in environments that may contain pets, secondhand smoke or airborne allergens.

• Active, unrestricted license(s) in the covered state(s).
• Board certification (required).
• Multi‑state license preferred.
• Per diem clinician experience or strong clinical assessment skills.

Hourly base rate of $37.66 – $81.11 with additional incentive structure per visit completed. Rates increase with experience, education, and geography.
