** Pay Rate:**  $ 47.37/hr.

** Responsibilities*
* + Coordinate patient care plans across providers, departments, and healthcare services.

+ Perform clinical triage through telephone, EMR messaging, and in-person patient interactions.

+ Review laboratory results, imaging studies, and other clinical findings, escalating concerns when appropriate.

+ Prepare patients for procedures and ensure all pre-operative requirements are completed.

+ Educate patients on diagnoses, treatment plans, medications, and follow-up care.

+ Coordinate referrals, authorizations, and specialty care services.

+ Accurately document all patient interactions and clinical activities within the electronic medical record (EMR).

+ Serve as the primary liaison between patients, families, providers, and the interdisciplinary care team.

+ Monitor patient progress, identify care gaps, and intervene to improve health outcomes.

+ Support quality improvement initiatives and promote best clinical practices.

** Required Qualifications*
* + Active, unrestricted Registered Nurse (RN) license.

+ Minimum of 2-3 years of recent clinical RN experience.

+ Strong clinical assessment and triage skills.

+ Excellent communication, organizational, and patient education abilities.

+ Ability to manage multiple priorities in a fast-paced outpatient environment.

** Preferred Qualifications*
* + 3-5+ years of RN experience.

+ Previous experience in Family Medicine or Primary Care.

+ At least one year of care coordination or case management experience.

+ Experience using Epic or another electronic medical record (EMR) system.

+ Proven ability to work effectively within multidisciplinary healthcare teams.

+ Experience managing high patient volumes and complex patient populations.
