Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) Case Manager/Care Coordinator for a temporary-to-hire opportunity with one of our premier healthcare clients in Houston, TX . This is an excellent opportunity for an RN who is passionate about care coordination, patient education, and improving patient outcomes in a collaborative outpatient setting.
The RN Case Manager will coordinate patient care across multidisciplinary teams, serve as the primary clinical point of contact for patients and families, and ensure seamless delivery of care throughout the patient journey.

Employment Type: Temporary-to-Hire
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Pay Rate: $ 47.37/hr.
ResponsibilitiesCoordinate patient care plans across providers, departments, and healthcare services.
Perform clinical triage through telephone, EMR messaging, and in-person patient interactions.
Review laboratory results, imaging studies, and other clinical findings, escalating concerns when appropriate.
Prepare patients for procedures and ensure all pre-operative requirements are completed.
Educate patients on diagnoses, treatment plans, medications, and follow-up care.
Coordinate referrals, authorizations, and specialty care services.
Accurately document all patient interactions and clinical activities within the electronic medical record (EMR).
Serve as the primary liaison between patients, families, providers, and the interdisciplinary care team.
Monitor patient progress, identify care gaps, and intervene to improve health outcomes.
Support quality improvement initiatives and promote best clinical practices.
Active, unrestricted Registered Nurse (RN) license.
Minimum of 2–3 years of recent clinical RN experience.
Strong clinical assessment and triage skills.
Excellent communication, organizational, and patient education abilities.
Ability to manage multiple priorities in a fast-paced outpatient environment.
3–5+ years of RN experience.
Previous experience in Family Medicine or Primary Care.
At least one year of care coordination or case management experience.
Experience using Epic or another electronic medical record (EMR) system.
Proven ability to work effectively within multidisciplinary healthcare teams.
Experience managing high patient volumes and complex patient populations.
