RN Healthcare Manager - WellMed at Singing Hills

The RN Healthcare Manager at Well Med is responsible for supporting patients with high-risk health conditions, acting as an educator, resource, and advocate to ensure maximum quality of life. • Pay: $60,200 - $107,400 annually

• Work with providers and clinic staff to identify patients at high risk.
• Support longitudinal care of patients with chronic conditions by:
  • Performing assessment of health conditions.
  • Performing medication reconciliation.
  • Conducting Motivational Interviewing and Self-Management Goal setting.
  • Providing patient education.
  • Creating referrals to appropriate agencies and resources.
• Support transition of patients with chronic conditions from inpatient to outpatient settings by:
  • Performing assessment of transitional needs.
  • Performing medication reconciliation.
  • Establishing and reviewing contingency plans.
  • Providing patient education.
  • Assisting with post-discharge needs such as prescriptions and appointments.
• Coordinate with providers to establish or update plans of care.
• Perform accurate and timely documentation in the electronic medical record.
• Participate in daily huddles and Patient Care Coordination (PCC) meetings.
• Prepare accurate and timely reports for weekly meetings.
• Maintain continued competence in nursing practice and knowledge of current evidence-based practices.
• Perform all other related duties as assigned.

Education:

• Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of experience as an RN.

Experience:

• 2+ years of experience in a physician's office, clinical, or hospital setting.

Licenses &

Certifications:

• Registered Nurse with an active and unrestricted license to practice in the state of employment.
• Current BLS certification required.
• This position requires Tuberculosis screening and proof of immunity to certain diseases.
