Pathways Hospice (Newest Start Up) is seeking a Hospice RN Case Manager (PRN) in Southeast Texas. The role focuses on patient assessment, care coordination, and advocacy to support patients and families at end of life.
The ideal candidate holds a valid Texas RN license (or Compact state) in good standing, with 1+ year in hospice case management. Experience with HCHB EMR is a plus, and certifications in hospice/palliative care are advantageous.
