As a Hospice RN Case Manager, you will lead and coordinate individualized patient care plans while delivering expert clinical support, comfort, education, and advocacy. You’ll partner with an interdisciplinary team to ensure every patient receives dignified, high-quality care throughout their hospice journey.
What You’ll Do- Complete initial and ongoing patient assessments
- Develop, implement, and update individualized plans of care
- Educate patients and families on disease progression and end-of-life support
- Coordinate care with physicians, social workers, chaplains, and clinical teams
- Supervise Hospice Aides and LVN/LPN support staff
- Maintain accurate clinical documentation and communication
- Participate in assigned on-call rotation
- Active RN license
- Current CPR certification
- Valid driver’s license and auto insurance
- Hospice RN experience preferred (2+ years strongly preferred)
- Home Care Home Base experience preferred
- Strong clinical judgment and compassionate communication skills
