Job Purpose:
- The Sharpview LPN - Wound Care at SOLUTIONS HEALTHCARE is responsible for providing high-quality wound care services to patients, ensuring optimal healing and recovery. This role involves assessing patient needs, implementing care plans, and collaborating with the healthcare team to deliver comprehensive wound management.
Key Responsibilities:
- Conduct thorough assessments of patients' wound conditions and document findings accurately.
- Develop and implement individualized wound care plans in collaboration with the healthcare team.
- Perform wound care procedures, including cleaning, dressing, and monitoring wounds, following established protocols.
- Educate patients and their families on wound care techniques and prevention strategies.
- Monitor and evaluate patients' progress, adjusting care plans as necessary to achieve optimal outcomes.
- Maintain accurate and timely documentation of patient care activities and progress.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care.
- Stay updated on the latest wound care techniques and best practices through continuous education and training.
- Adhere to all healthcare regulations and standards to ensure patient safety and quality of care.
- Participate in quality improvement initiatives to enhance wound care services and patient satisfaction.
Required
Education:
- Completion of a state-approved Licensed Practical Nurse (LPN) program.
- Current and valid LPN license in the state of practice.
Required Experience:
- Minimum of 2 years of experience as an LPN, preferably in a wound care or similar healthcare setting.
- Proven track record of working effectively in a multidisciplinary healthcare team.
- Experience in assessing, planning, and implementing wound care treatment plans.
Required
Skills and Abilities:
- Strong knowledge of wound care practices, including infection control and prevention techniques.
- Excellent communication skills to effectively interact with patients, families, and healthcare team members.
- Ability to perform wound assessments and document findings accurately.
- Proficient in the use of electronic health records (EHR) for documentation and patient care management.
- Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
- Compassionate and patient-centered approach to care, with a focus on improving patient outcomes.
- Ability to maintain confidentiality and adhere to ethical standards in all aspects of patient care.
