The Case Manager Navigator at MD Anderson Cancer Center integrates and coordinates utilization management concepts, care facilitation, and discharge planning functions. This role directly impacts patient care, satisfaction, and institutional outcomes by supporting complex transitions and ensuring that patients receive the right care at the right time throughout the continuum.

The ideal candidate will have a Bachelor's Degree in Nursing (BSN) and five years of professional nursing experience, with the option to reduce the requirement to four years if one year has been spent in oncology nursing or case management. The candidate must hold a current Texas RN license, maintain American Heart Association Basic Life Support (BLS), and ideally hold Certified Case Manager (CCM) or Advanced Case Manager (ACM) certifications.

The ideal candidate must also be available to work primarily weekends.

Schedule:

Four 10 hour shifts, two of which must be weekend shifts, with flexible weekday scheduling, 8:00 AM - 6:30 PM, on site.

Case Management at MD Anderson ensures comprehensive care coordination, discharge planning, resource management, and patient advocacy for high risk and complex patients. Navigators collaborate closely with physicians, nurses, social workers, payors, and community agencies to optimize patient outcomes and patient experience.

Hourly breakdown of salary range:
Minimum $48.56 | Midpoint $60.82 | Maximum $73.08

Essential Job Functions:
  • Assess individual patient needs based on practice models for high risk or complex care issues
  • Coordinate, facilitate, and monitor care across the continuum to ensure optimal clinical, functional, social, cognitive, satisfaction, and financial outcomes
  • Oversee discharge planning and facilitate safe transitions to community settings
  • Intervene, advocate, and facilitate care plan adjustments on behalf of patients
  • Act as a liaison for multidisciplinary teams, external physicians, payors, and community agencies
  • Identify and implement strategies to improve processes, reduce costs, and enhance patient care and business outcomes
  • Other duties as assigned

Education Required:

Bachelor's degree in Nursing

Experience

Required:

Five years professional nursing experience; may reduce to four with one year in oncology or case management; or substitute BSN with ADN plus seven years of nursing/case management/navigation

Licensure/Certification

Required:

Current State of Texas Professional Nursing License (RN);
American Heart Association Basic Life Support (BLS)

Licensure/Certification Preferred:
