Summary:

We are seeking an experienced and relationship-driven Hospice Clinical Liaison to serve as a trusted clinical resource to hospitals and other healthcare partners.

• You may be a great fit for this role if you are a nurse who enjoys connecting with people as much as providing clinical care. You understand that excellent hospice outreach is not simply about generating referrals- it's about earning trust, providing meaningful education, recognizing when hospice may help, and making it easier for patients and families to access compassionate care when they need it most.
• You are proactive, responsive, comfortable starting conversations, and able to develop lasting professional relationships while maintaining the clinical integrity that hospice care requires.

Qualifications:

• Graduate of an accredited school of nursing with a current Texas license as a Registered Nurse.
• Current certification in basic life support.
• Excellent organizational skills to handle a variety of tasks simultaneously.
• Effective interpersonal skills.
• Excellent communication skills, both oral and written.
• Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
• Ability to function as part of an inter-disciplinary team.
• Ability to travel to multi-work locations during workday.
• Must have reliable, insured vehicle and valid Texas Driver's license.

Duties/Responsibilities:

• Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, skilled nursing facilities, assisted living communities, and other healthcare partners
• Serve as a clinical resource and subject matter expert regarding hospice philosophy, services, eligibility criteria, and the referral and admission process
• Identify patients who may benefit from hospice services and collaborate with referral sources to facilitate timely evaluation and referral
• Review available clinical information to help determine potential hospice appropriateness and coordinate additional clinical review when needed.
• Educate patients, families, caregivers, and healthcare professionals about hospice care, available services, and what to expect throughout the admission process.
