A strong background in patient assessment, care coordination, clinical documentation, research collaboration, communication, and procedural skills is preferred. Responsibilities:
Inpatient/Outpatient Activities    Conducts inpatient rounds with attending physicians and communicates pertinent clinical information based on physical examination and review of patient data.  Evaluates patients and communicates pertinent clinical information based on history, physical examination, and review of patient data to attending physicians.  Facilitates appointments and testing for new and consultation patients within the department.  Manages daily patient problems and emergency situations as they arise and documents actions implemented and their resolution.  

Completes PMEs, admission H&Ps, consultation notes, daily progress notes, and discharge summaries in compliance with institutional and departmental guidelines.  Requests consultations and communicates with other services, caregivers, and outside providers to coordinate patient care.  Coordinates admissions as needed by the patient and as dictated by training.  Conducts hospice referrals and coordinates discharge…
