RN II: Care Management & Transitional Care

Horizon Blue Cross Blue Shield of New Jersey is seeking an RN to perform case management duties for the Primary Nurse population. The role focuses on planning transitions to the continuum of care and developing member-centric plans of care.

You will outreach to high-risk members and engage them in preventative care opportunities and screenings. As a mentor to new RNs and staff, you will coordinate care with providers, facilities and families, while traveling to on-site locations as needed.
