- Build and strengthen relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other referral partners
- Develop new referral sources while maintaining and growing existing accounts
- Represent Choice Health at Home as the preferred provider for exceptional patient care
- Meet and exceed monthly admission and census growth goals
- Partner closely with clinical teams to ensure smooth patient transitions and quality outcomes
- Execute strategic territory planning and community outreach initiatives
- Maintain compliance with all healthcare marketing regulations and agency standards
- Recent home health or hospice marketing/sales experience required
- Proven success growing referrals and building strong healthcare relationships
- Existing physician and referral source relationships preferred
- Self-motivated, goal-oriented, and relationship-driven professional
- Excellent communication and presentation skills
- CRM and Home Care Home Base experience preferred
- Valid driver’s license and auto insurance required
