Job Description

Under the supervision of the Regional Program Manager, provides advanced and comprehensive case management consultation for families with children who have health risks, conditions or special health care needs enrolled in or applying for the following programs:
Children with Special Heath Care Needs (CSHCN), Supplemental Security Income (SSI), Newborn Screening (NBS), Newborn Hearing Screening (NBHS), Childhood Lead Poisoning Prevention (CLPPP), Personal Care Services (PCS), Community First Choice (CFC) and Case Management for Children and Pregnant Women (CPW). Assists families with the eligibility determination process for CSHCN. Conducts home visits to determine eligibility, complete family needs assessments and service plans and also follow-up on services rendered to families on caseload.

Connects families to community resources. Participates in community events while communicating program services and/or changes to consumers and the public in a professional and accurate manner. Provides training, consultation and technical assistance to program contractors and program providers. Actively participates and advocates in Community Resources Coordination Groups (CRCG), case management coalitions and community organizations. Provides information and referral services. Works under minimum supervision with considerable latitude for the use of independent judgment.

Maintains confidentiality. Other duties as assigned include but are not limited to actively participating in public health needs including but not limited to disaster, biological, environmental and weather emergencies.

Essential Job Functions

  • 40%- Case Management Functions – Provides complex and specialized social work consultation and comprehensive case management services to families who receive services or are seeking services from the CSHCN, SSI, NBS, NBHS, CLPPP, and CPW programs. Conducts home visits for comprehensive assessment of home environment and client needs. Following program guidelines, provides comprehensive case manager services including the intake, family needs assessment, service plan, and follow up services.

    Assists families with the eligiblity determination process for CSHCN. Provides information and referral services, linkage to community resources, family supports, advocacy, and service coordination. Maintains documentation of work activities with record documentation, weekly reports, travel reports, case summaries, and correspondence. Utilizes software programs to do database entry, word processing, spreadsheets, electronic mail, and internet.
  • 20%- Conducts comprehensive assessments to determine eligibility and need for PCS. Documents identified needs and forwards documentation for authorization of PCS. Conducts reassessments for PCS as required or as necessary according to family needs and policy…
