Housing Case Manager - Supportive Housing Program

Location: Houston, TX (Gulf Freeway area)

Pay Rate: $26.00 per hour (paid weekly)

Make a direct impact on the lives of young people building their path to independence. This Housing Case Manager role supports youth ages 18-24 as they move through a supportive housing continuum and into stable housing, while developing the skills needed for long-term success.

Openwork is partnering with a community-focused organization to hire for this meaningful opportunity.

This role combines hands-on housing navigation with ongoing, strengths-based case management to help youth achieve housing stability and confidently transition into adulthood.

Position Overview:

The Housing Case Manager works closely with youth as they navigate the housing and leasing process, including housing voucher applications, required documentation, and housing briefings. Once youth are housed, the Case Manager provides consistent, individualized support to help them maintain housing, overcome barriers, and work toward greater independence.

Key Responsibilities:
  • Conduct housing readiness assessments and provide individualized case management and supportive services.
  • Guide youth through the housing application and voucher process, including completing required forms and attending housing briefings.
  • Assist youth with submitting documentation to local housing authorities, including:
    • Initial applications
    • Recertifications
    • Move requests
    • Income changes
    • Family composition updates
  • Support youth through the move-in process to ensure a smooth transition into housing.
  • Provide ongoing onsite case management to youth residing in supportive housing.
  • Participate in wraparound meetings to coordinate care and support services.
  • Maintain accurate, up-to-date case files in a case management database system.
  • Collaborate closely with internal programs to ensure youth understand available housing options and how to prepare.
  • Attend and participate in program events and workshops as needed.
Required Education:

Bachelor's degree in Social Services, Psychology, Social Work, Sociology, Human Services, Criminal Justice, or a closely related field

Required Skills:
  • Proficiency in Microsoft Word, Excel, Outlook, and data entry systems.
  • Strong organizational and documentation skills..
  • Ability to build trusting, supportive relationships with youth.
  • Effective communication and teamwork skills.
Preferred Experience:
  • Knowledge of case management principles, documentation standards, and records management.
  • Experience with or knowledge of public housing programs and housing subsidies.
  • Experience working with youth aging out of foster care or navigating transitional housing.
Schedule:

Monday-Friday; occasional weekends as discussed during the interview process

Work Environment & Requirements:

This role involves regular, in-person engagement with youth in a supportive housing setting and requires flexibility, sound judgment, and collaboration across teams.
  • Reliable transportation required.
  • Valid driver's license and current liability insurance required.
