  • Conduct initial assessments and intake interviews with new residents to determine their needs and develop individualized care plans
  • Monitor and evaluate residents' progress, adjusting care plans as necessary
  • Provide ongoing support and counseling to residents to address their physical, emotional, and social needs
  • Coordinate with healthcare providers, social workers, and other professionals to ensure comprehensive care
  • Maintain accurate and up-to-date case files and documentation
  • Assist residents in accessing community resources such as education, employment, and recreational programs
  • Develop and facilitate life skills training sessions for residents
  • Advocate for residents' rights and needs within the community and service systems
  • Ensure compliance with all relevant regulations and organizational policies
  • Participate in team meetings, case conferences, and training sessions
Residential Case Manager

Qualifications & Skills
  • Bachelor's degree in social work, psychology, or a related field
  • Previous experience in residential case management or a similar role
  • Excellent interpersonal and communication skills
  • Ability to work effectively in a team environment
  • Knowledge of community resources and services
  • Proficient in case management software and Microsoft Office applications
  • Bachelor's degree in social work, psychology, human services, or a related field
  • Minimum of 2 years of experience in case management or a related field
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and professional boundaries
  • Valid driver's license and reliable transportation
  • CPR and First Aid certification
  • Ability to work flexible hours, including evenings and weekends, as needed
  • Strong problem-solving skills and the ability to make sound decisions under pressure
  • Empathy, patience, and a genuine desire to help others
