Fifty years later, Harris County Resources for Children and Adults still provides this support. But the now-county department has expanded services for early prevention, intervention, and adult services.

Brief

Position Overview:

Under the supervision of the Project/Program Supervisor, the Housing Campus Case Manager will work with youth ages 18 through 24 who are referred to The HAY Center Housing Continuum for housing case management services. This position is responsible for assessing and addressing each youth's unique housing needs and ensuring that youth and young adults achieve housing stability by providing crisis intervention as needed, including connecting youth to appropriate resources and supportive services.

Duties & Responsibilities:

* Provides housing case management to former foster youth transitioning to independence, including lease review and explanation, home visits, and rent ledger monitoring to ensure payment compliance. Assesses housing stability through review of intake assessments, service plans, and budgets, and delivers life skills training and connection to community resources. Advocates with landlords, property management, and housing authorities to prevent evictions and support long-term housing stability.

* Completes required case documentation in a case management database system and maintains case files for assigned cases to ensure that all information regarding the youth's housing plan is current and updates are available to the HAY Center team.

* Assists youth in completing and submitting required documentation to local housing authorities throughout the housing process, including initial applications, recertifications, move requests, income changes, and changes in family composition, to ensure ongoing compliance with housing program requirements.

* Attends and participates in wraparound meetings to provide updates on youth housing goals and progress, and collaborates with the youth and HAY Center program staff to complete action items supporting housing stability and overall goal attainment.

* Attends and participates in all HAY Center and Housing Continuum events and workshops that includes Personal Advocacy Meetings.

* Attends and participates in life skills training class - Housing and Transportation to review housing readiness with youth in care. These events may take place on some weekends.

Requirements

Education:

* Bachelor's degree in Psychology, Sociology, Social Work, Criminal Justice, or relevant field from an accredited college or university

Experience:

* Two (2) year of verifiable full-time paid case management experience (including case documentation) working in an educational, institutional, counseling, or social service setting working with youth who have challenging life circumstances.

* Experience in working with housing providers including HUD and privately funded housing options.

* Training and experience using a personal computer with a variety of software, including Microsoft Office.

Licensure:

* Reliable transportation with a valid Texas driver's license and auto liability insurance.

* Ability to become CPR/First Aid certified.

Knowledge, Skills & Abilities:

* Knowledge and understanding of adolescent/transition age youth stages of development and the transitional needs including life skills and linkages of youth aging out of the foster care system

* Must possess good assessment and intervention skills.

* Ability to problem solve and demonstrate critical thinking skills
